Athletic Forms and Documents
Student-athletes must complete all required registration, medical, and participation forms before taking part in practices or competitions. Families should review the documents below and submit completed forms according to the instructions provided by the Athletic Department.Required Forms

Physical Examination Requirements
A current physical examination must be on file before a student may participate in athletics. Incoming freshmen should complete the State of Illinois school physical form, which may also satisfy the athletic physical requirement. An IHSA sports physical alone does not meet the required school health examination requirement for students entering ninth grade
Families should contact the Main Office or Athletic Department with questions about physical expiration dates, required documents, or submission procedures.
