After your application is received, a member of the St. Teresa admissions team will contact your family regarding the next steps. Depending on your student’s grade level and previous school, we may request academic records, standardized test results, or other supporting information.

Health Forms
Illinois requires students entering ninth grade to submit a completed physical examination form. An athletic physical does not fulfill the state requirement for the ninth-grade school health examination.
Additional dental or health records may also be required based on the student’s grade level or individual circumstances.Tuition Assistance Is Available
